  Fillers

    When there is a fold or a depression in the skin (whether a wrinkle or a scar), using a filling agent might be the best approach. The most common problems treated with fillers are nasolabial fold wrinkles (the line running from the nose to the corner of the mouth),  the lips and fill in subtle contour defects on the cheeks and chin. It also can be used at the corner of the mouth to correct the sagging/sad expression that often appears with advanced age. Fillers can also be used to fill in scars left by acne or other problems.

  What filling agents can be used?

    

Fillers can be divided into injectable products and implantable ones.

 

Dr. Touma's favorite fillers are hyaluronic acid,  RADIESSE and lipofilling, i.e. using one's own fat.

 

Hyaluronic acid is a material found in the mid portion of the skin where collagen and elastin are found native. Hyaluronic acid  helps to give skin its substance and integrity and is diminished in the aging process. Injectable hyaluronic acid is a non-animal derived gel like substance and does not need any allergy test before treatment, unlike collagen, and has been found to last 6-8 months on average. It is a biodergradable product that looks like a clear gel and is injected under skin lines, folds, scars or lips. When used properly it lasts up to 8 months, and is extremely safe with problems occurring in less of 1/5000 injections. (Examples of this product are JUVEDERM . Hyaluronic acid is approved by the United States FDA. Large volumes of hyaluronic acid can also be used to enlarge buttocks or other body areas.
RADIESSE is a newer FDA approved product based on calcium hydroxyl-apatite. It is resolbable after 1-2 years. It is best used to correct nose deformities and for cheek augmentation.

 

When longer lasting results are desired and large volumes are needed, Dr. Touma prefers to use lipofilling. Lipofilling is a multi-step procedure that requires microliposuction to harvest fat from an area of excess on the body local anesthesia to the recipient area, usually the face, and then reinjecting the fat into that area.
This is a highly technique dependent procedure, because it requires great skill in order to inject the fat in minuscule particles into multiple lagers of the skin. Swelling and some redness are expected after the procedure. Dr. Touma has a long experience using the best techniques of lipofilling, and after sees results lasting over 12 years.

Implantable products such as Goretex  are sometimes used as a filling agent but require a more extensive procedure. This biocompatible synthetic material has been used for over 20 years in cardiovascular surgery and hernia repair. More recently, it has found its place in cosmetic surgery as an excellent material for permanent soft-tissue augmentation.

 

Goretex is implanted immediately under the skin through very small incisions. Unlike hyaluronic acid , local anesthesia is needed. The material is not biodergradable and results are permanent. In addition, no allergic reactions have been reported.

Goretex is best used to augment nasolabial folds, marionette lines, thin lips, and some depressed scars.

  Lip Augmentation

    Lips are best treated with Hyaluronic acid or lipofilling. For a permanent augmentation, the new PERMALIP, an implantable silicon tube in the shape of the lip is now recommended. Like breast silicone implants, this is a safe and durable choice.

  Important note

    

When having injections into the skin it is recommended to use the safest products available, even when the results are a little less durable, because if there is a problem with the product removal of the product is difficult and sometimes impossible. Hyaluronic acid and lipofilling are the safest fillers available.

 

Also, it is important to remember that the aging process is continuous and that fillers, no matter how long lasting, will not have a constant correction over time.
